JOB TITLE: Community Health Coordinator

DEPARTMENT: Programs

SALARY: $42,000-45,000

SUMMARY: Provides bi-lingual health coaching and social services navigation to low-wealth individuals and families.


DUTIES AND RESPONSIBILITIES:

  • Provide one-on-one health coaching, public benefits enrollment, and access to health care services.
  • Facilitate and conduct group health education classes/workshops.
  • Identify and refer participants to resources and services needed to achieve health goals.
  • Coordinate and participate in community events and meetings related to health outreach days throughout Miami-Dade County.
  • Link individuals and families to medical and social services.
  • Maintain client/participant information and data in organization’s data tracking and management systems.

SUPERVISORY RESPONSIBILITIES:

  • This job does not directly supervise employees.

QUALIFICATIONS:

  • Language: Professional working proficiency in oral and written English. Professional working proficiency in oral and written Haitian Creole.
  • Education: Associate’s Degree
  • Years of Experience: 0-3
  • Certification or License: Certification in the Department of Children and Families Florida, Florida Certified Marketplace Navigator
  • Training: HIPPA, ACCESS Civil Rights, Security Awareness (or must be attained with 60 days of hiring)
  • Computer skills: Microsoft Office Suite and working knowledge of web-based platforms

PHYSICAL DEMANDS AND WORK ENVIRONMENT:

  • Ability to lift weights of up to 25 pounds
  • Stretch and bend to organize and distribute materials for Catalyst Miami activities
  • Able to sit at a desk for long periods of time and utilize a computer for extended periods
  • Must be able to work flexible hours to accommodate project needs
